Popis: Introduction: odontogenic keratocyst has histopathological significance, with interesting biological and pathological behavior. Its high recurrence rate and great capacity to destroy areas of medullary bone make it one of the most aggressive cysts. Objective: to characterize the diagnosis and update treatment of odontogénico keratocyst. Method: an updated bibliographic review on the subject was carried out in the period between February and April 2021. The search used textbooks and the resources available on the Infomed network, specifically: PubMed, SciELO and Ebesco, through databases: Medline, Academic Search Premier, MedicLatina; in addition to Cudmed, Lilacs and Scopus. From a total of 56 articles related to the topic, 24 were selected according to date and relevance. Development: keratocysts are asymptomatic, founds by chance during radiographic examinations. The therapeutic approach should be based on the patient's age, lesion location, cyst size, recurrence status, and radiographic evidence of cortical perforation. Conclusions: radiology is, without doubt, the first method used in the diagnosis of cysts; however, the histopathological analysis is in charge of establishing the definitive diagnosis. Several are the treatments described in the literature; excision and curettage are essential in cases, always giving the patient a clinical and radiographic follow-up for a period of 5 years.
