Desmoplastic small round cell tumor of the kidney

Popis: A 14-year old girl, without previous medical history, complained of a diffuse swelling of the left abdomen present since a few months. She consulted a general practitioner. She was told to have an asymmetric abdominal development due to an asymmetric inferior limb length. After an abdominal trauma she suffered from heavy abdominal pain and vomiting and underwent a multiphase CT after intravenous contrast injection. The CT scan showed a huge, lobulated heterogeneous solid mass, probably from renal origin with central low attenuation regions compatible with necrosis of hemorrhage and multiple foci of punctuate calcifications. The mass displaced the surrounding structures. Multiple nodular masses were demonstrated in the lung and two hypodens lesions in the liver suggestting metatases. There was no ascites of enlarged lymph nodes. MRI of the abdomen was performed wihich showed few normal renal parenchyma surrounding the tumor and pointing to its renal origin. The differential diagnosis of a Wilm's tumor and a renal cell carcinoma was proposed taking into account the age, the renal origin and the imaging findings. A percutaneous biopsy of the mass was performed and microscopic analysis and immunohistochemical reactions resulted in the diagnosis of the desmoplastic small round cell tumor.
