Popis: Drug therapy is becoming more complex. The patients need more potent and more sophisticated drug therapy. To ensure safe medication use, health professionals must be aware of the drug administration. A medication error may be defined as an unintended act (either of omission or commission) or as an act that does not achieve its intended outcome. Some common causes of medication errors are: lack of the knowledge of the drugs, lack of information about the patients, failed communication, poor drug distribution practices, dose miscalculation, incorrect drug administration, lack of patient education. In all cases, the causes are multifactorial, cutting across many lines of responsibility. Although, a few drugs have a high risk of causing injury when they are misused. This high-alert drugs can be targeted for specific error reduction interventions. The paper also suggests many actions that pharmacist can take to reduce or eliminate medication errors. Developing internal systems for reporting and tracking errors is the first step toward identifying problems.
