MULTIPLE BURNS IN A 22-MONTH-OLD CHILD: CASE REPORT

Popis: Više od 120.000 djece godišnje dolazi u hitnu pomoć zbog opeklina. Većinom je riječ o manjim opeklinama koje ne zahtijevaju bolničko liječenje. Prikazali smo hitno zbrinjavanje i liječenje djevojčice u dobi od 22 mjeseca s opeklinama dišnog puta i višestrukim opeklinama kože tijela. Djevojčica je dovežena kolima hitne pomoći u Klinički bolnički centar Rijeka iz Poreča, gdje je požar u kamp kućici uzrokovao višestruke opekline. U početnoj procjeni utvrđena je opeklina dišnog puta, stridorozno i insufi cijentno disanje te hemodinamska nestabilnost. Djevojčica je transportirana u operacijsku dvoranu gdje je odmah zbrinut dišni put te osiguran intravaskularni pristup. Procijenjena je opečena površina tijela koja je iznosila oko 35 % ukupne tjelesne površine. Započeta je nadoknada volumena te su opekline kirurški zbrinute. Daljnje liječenje se nastavlja u Jedinici intenzivnog liječenja KBC Rijeka. Do jutarnjih sati se stanje djevojčice pogoršava pa ga se pokuša korigirati u konzultaciji s pedijatrom-intenzivistom. U daljnjem tijeku liječenja zdravstveno stanje djevojčice se poboljšava i postaje vrlo dobro, opekline su redovito previjane te se bolesnica premješta na odjel kirurgije.
Over 120,000 children present to emergency department due to burns annually. Many burn injuries are minor and do not require hospital admission. We report a case of airway burns and major body burns in a 22-month-old girl. Emergency medical service brought the girl to the Rijeka University Hospital Centre from Poreč, where she had suffered burns from the fi re in a car. Initial assessment revealed upper airway burns that presented with stridor, breathing was insuffi cient, and she was hemodynamically unstable. The patient was immediately transferred to the operating room where the airway was secured and an intravascular route was obtained. Estimated burn size was approximately 35% of the total body surface area; volume resuscitation was started and burns were surgically revised. After the operation, the patient was transferred to the Hospital Intensive Care Unit. Until the morning, her condition worsened and in consultation with the pediatrician correction of the disorders was done. During further clinical course, her health condition improved very well, the burns were routinely revised, and she was transferred to surgical ward.
