Popis: Benigna hiperplazija prostate (BPH) jedno je od najčešćih stanja koje pogađa muškarce i povezana je sa simptomima donjeg urinarnog trakta/LUTS. Terapija BPH uključuje aktivno praćenje, medikamentoznu terapiju te kirurško liječenje. Kada dođe do razvoja komplikacija kliničke BPH u vidu opstrukcije na izlazu iz mokraćnog mjehura zbog benigne opstrukcije prostate, terapija lijekovima često nije dostatna te postoji potreba za kirurškim metodama liječenja. Kirurške metode uključuju transuretralnu resekciju prostate, transuretralnu enukleaciju i resekciju prostate, transuretralnu inciziju prostate, otvorenu prostatektomiju, transuretralnu vaporizaciju prostate, transuretralnu vaporizaciju-resekciju prostate, transuretralnu mikrovalnu termoterapiju, transuretralnu ablaciju iglom, intersticijsku lasersku koagulaciju prostate, Ho:YAG lasersku enukleaciju prostate te KTP lasersku vaporizaciju prostate. Iako se transuretralna resekcija prostate smatra zlatnim standardom te sigurnom i učinkovitom metodom u kirurškom liječenju BPH, može imati određene komplikacije kao što su krvarenje, sindrom transuretralne resekcije te nepotpunu resekciju. Razvojem novih tehnologija kao što su laserske tehnike, enukleacijske tehnike te bipolarne transuretralne resekcije ti su nedostaci smanjeni. Provedena su brojna kratkoročna i dugoročna istraživanja usporedbe transuretralne resekcije prostate s drugim metodama te su utvrđene određene prednosti i nedostaci pojedinih tehnika. I dalje su otvorena prostatektomija i transuretralna resekcija prostate metode izbora u liječenju BPH.
Benign prostatic hyperplasia (BPH) is one of the most common conditions affecting men and is associated with lower urinary tract symptoms/LUTS. BPH therapy includes active monitoring, drug therapy, and surgical treatment. When complications of clinical BPH develop in the form of bladder outlet obstruction due to benign prostate obstruction, drug therapy is often insufficient and there is a need for surgical treatment. Surgical methods include transurethral resection of the prostate, transurethral enucleation and resection of the prostate, transurethral incision of the prostate, open prostatectomy, transurethral vaporization of the prostate, transurethral vaporization-resection of the prostate, transurethral microwave thermotherapy, transurethral ablation with needle, interstitial laser coagulation of the prostate, Holumium laser enucleation of the prostate and Potassium-titanyl-phosphate laser vaporization of the prostate. Although transurethral resection of the prostate is considered the gold standard and a safe and effective method in the surgical treatment of BPH, it can have certain complications such as bleeding, transurethral resection syndrome, and incomplete resection. With the development of new technologies such as laser techniques, enucleation techniques and bipolar transurethral resections, these shortcomings have been reduced. Numerous short-term and long-term studies comparing transurethral resection of the prostate with other methods have been conducted, certain advantages and disadvantages of certain techniques have been identified. Still, open prostatectomy and transurethral resection of the prostate are method of choice for treatment of patients with BPH.
