This bachelor's thesis focuses on the process of planning the patients discharge from acute care to after care to provide the best care possible with focus on the need of the inpatient. The first part of the thesis explains the history and present state of connection between social and health areas but mainly deals with the position of medical social worker who connects both areas and who cares about discharge planning.
