Popis: | Nowadays, as in many other sectors, digital transformation has encountered its place within health care ecosystem. However, players in the healthcare industry struggle to successfully manage the myriad stakeholders, regulations,and privacy concerns required to build a fully integrated healthcare IT system. Patients around the world have grown more comfortable using digital networks and services, even for complex and sensitive issues such as health-care, but one of the root causes for not unlocking full digitization adoption resides in health data fragmentation. Being able to unified the multitude sources of medical information and data in a standardized and useful format can trigger the delivering of real value. Healthcare digitization can benefit from the constant presence of smartphones in our daily lives. With its data connectivity and multiple intrinsic sensors they become a decisive tool for healthcare data systems integration, which in turn, can boost making sense of health data to patients and clinicians as well as to the overall of healthcare stakeholders. This project consists in the development of Health Track system, an end-to-end and patient oriented solution that gathers multiple types of patients data and turns them into meaningful outcomes. On the other hand, as the patient remains at the center of Health Track environment, it was found important to tackle through the implemented solution a worldwide population issue concerning Noncommunicable Diseases (NCDs). According to the World Health Organization, 3 out of 5 deaths worldwide are due to common, chronic conditions, such as heart and respiratory diseases, cancer, and diabetes. These Noncommunicable Diseases (NCDs) are linked to multiple lifestyle risk factors, including smoking, the harmful use of alcohol, and physical inactivity. Hence, providing a tool to motivate and empower individuals to make the behavioral and choice changes needed to improve their healthand reduce NCD risk factors became a key goal of this project Objectius de Desenvolupament Sostenible::3 - Salut i Benestar |
