Popis: | Background: a long interpregnancy interval has been associated with risks of adverse obstetrical and perinatal outcomes. Labour processes seem to be different with longer labour and more functional dystocia. Preterm births, low birth weights and pregnancy diseases appear to increase. Very few studies focused on this subject and none have yet to be conducted in France. Objectives: primary objective: describe the labour process after a long interpregnancy interval. Secondary objectives: study maternal and neonatal morbidity associated with a long interpregnancy interval. Study design: a retrospective exposed-unexposed cohort study was conducted at the tertiary unit of Robert Debré in Paris between January 2000 and March 2016. 1 281 births were included with new living singleton with0 cephalic presentation after an interpregnancy interval of more than 59 months (exposed). We compared pregnancy, delivery and neonatal outcomes of this group to a group with an interval between 18 and 23 months (unexposed). The results were adjusted according to maternal age, parity, body mass, obstetrical history and pregnancy outcomes. Main results: we did not find any difference in duration of labour (4h39 vs 4h35) and perineal damages (OR=0,99 [0,79;1,24]) between the two groups. Prematurity risk (OR=1.70 [1.08;2.66]), caesarean sections rate (OR=1.46 [1.07;2.00]) and instrumental extraction rate (OR=1.59 [1.16;2.17]) were increased in the exposed group.
