Vocal Cord Paralysis Due to Post-Viral (COVID-19) Vagal Neuropathy Assessed by Transcranial Magnetic Stimulation

Popis: The submitted abstract presents a unique report of a patient with paralysis of the left vocal cord examined with navigated transcranial magnetic stimulation (TMS). TMS examination showed to be useful in the assessment of corticobulbar tract integrity by recording motor-evoked potentials (MEP) from vocal muscles. MEP response represents the final pathway of spatial and temporal summation of several descending volleys activating the α-motoneurons, thus reliably reflecting the excitation state of the cortico-bulbar cells for the vagal nerve, the corticobulbar tract, the peripheral recurrent laryngeal nerve, and the target vocal muscle. The radiological absence of CNS, neck, and thorax processes and no history of endotracheal intubation with positive antibody testing suggests that unilateral left vocal cord paralysis was due to post-viral (COVID-19) vagal neuropathy. TMS examination proved to serve as an additional armamentarium in clinical laryngological assessment.
