In 22 patients with hemodynamically significant pulmonary embolism CO2 at the end of expiration (ETCO2) was measured. Alveolar-arterial CO2 gradient (A-aCO2) and physiological dead space were also determined. In majority of patients ETCO2 was reduced. A-aCO2 gradient was elevated, and ventilation of dead space increased. Highly positive linear correlation between pulmonary artery pressure (MPAP) and A-aCO2 was also found (r=0.81). Relation between MPAP and Dphys was similar (r=0.77). Highly negative linear correlation was found between cardiac index (CI) and Dphys (r=0.89). Results of the study indicate that a simple and noninvasive procedure of capnography can more reliably establish the diagnosis of pulmonary embolism and determine the severity of the disease. |
