Cosyntropin for Treatment of Chronic Inflammatory Demyelinating polyneuropathy (CIDP)

Popis: CIDP is manifested by inflammatory myelin lesion primarily in the roots and proximal segments of peripheral nerves. The cause is not known. Immunological studies suggest that it is, at least partially, a T-cell-mediated disorder. The disease is either chronic of intermittent with exacerbations. CIDP is diagnosed by clinical, electrophysiological, and histopathological findings, as well as cerebrospinal fluid (CSF) analysis. Immunomodulating steroids, plasma-exchange, or intravenous immunoglobulins are considered to be effective treatments. We present a case of the hypertropfic variant of CIDP combined with CNS demyelinization in a patient followed for 29 years. The administration of cosyntropin, a synthetic preparation of adenocorticotropic hormone used primarily to test adrenocortical response or reserve, during relapses has always resulted in excellent remissions. We conclude that cosyntropin has its place in CIDP treatment. The results of its application correlate with those obtaindes with much more exprensive medications and/or procedures.
