Impact of treatment of the gestational hypertension with methyldopa or nifedipin retard on dopplersonographic parameters

Popis: Aim: To investigate the effect of treatment of the gestational hypertension with methyldopa or nifedipine retard on Doppler sonography hemorrheological parameters. Material and Methods: The study included a total of 72 pregnant women with gestational hypertension (BP> 140/90 mmHg without proteinuria), with no other comorbidities and without differences in age, gestational age and parity. 52 pregnant women were orally treated with methyldopa, and 20 pregnant women were taking calcium channel blocker nifedipine in retard form. Changes in hemodynamics during treatment were observed with Doppler ultrasound measurements ofthe maternal circulation (a. uterina) and fetoplacental circulation (a. umbilicalis, a. cerebri media). Fetal vasocentralisation is objectified with inversion of cerebroumbilical ratio. Results: Pregnant women who consumed retard nifedipine had significantly more frequent pathological doplersonografic findings of maternal and fetoplacental hemodynamics before birth (p 0.532: 0.026). Cerebroumbilical value ratio at the beginning and end of hypertentsive medication treatment in both groups were not significantly different. Conclusion: According to this study, better effects on hemodynamic parameters ultrasound maternal and fetoplacental circulation has methyldopa treatment.
