Introduction: Neurally mediated syncope (NMS) is a transient loss of consciousness (TLOC) usually provoked by orthostatic stress. It represents a dysfunction of the autonomic nervous system. Its occurrence in supine position is rare. We present a case of NMS, cardioinhibitory type with asystole, provoked by cannulation while patient in supine position. Case report: Female patient was admitted for evaluation of recurrent short-lasting transient loss of consciousness (TLOC) that occurred after prolonged standing, in stressful or painful situations. TLOC was presided by a feeling of warmth, nausea and lightheadedness. A head-up tilt-table test was proposed for NMS diagnosis. At the start of the test, while the patient was in the supine position, cannulation of the right cubital vein was performed. The patient immediately started to feel prodromal symptoms that eventually led to TLOC and she was unresponsive for few seconds with her head deviating to the left and myoclonic jerks of all four extremities. The continuous fall in the heart rate and blood pressure were registered with beat-to-beat EKG and blood pressure recording. An asystole lasting for 3 seconds was registered with unmeasurable blood pressure at the same time. Patient’s recovery was fast and complete without any specific therapy. The diagnosis of NMS, cardioinhibitory type with asystole was made. Conclusion: NMS can occur in supine position and head-up tilt table test with beat-to-beat EKG and blood pressure recording is a useful test in its diagnosis. |
