THE ROLE OF BIPOLAR RESECTOSCOPY (VERSAPOINT) IN TREATMENT OF BENIGNANT ENDOMETRIAL CHANGES

Popis: Since March 2004 in Department of Gynecology and Obstetrics of Požega County General Hospital we started to perform bipolar resectoscopy as method of choice for treatment of benignant endometrial changes (i.e. endometrial polyps, submusocal myomas, recurrent menopausal metrorrhagia, uterine cavity adhesions and septums etc.). We are using Versapoint (Gynecare, Johnson & Johnson) operative hysterosocope (resectoscope) of 27 Fr. diameter with microelectrodes fit through the 5 Fr. operating channel. Loop electrode is more convenientfor largertissue structures when it is necessary histological verification (polyps, myomas, endometrial Slation), but 0- degree vapor electrode is more pradical for vaporization, when is no need for bioptical material 'lesions, septums). For example, septum can be dissected by each of those electrodes, because both ofthem 'porize, cut and coagulate tissue structures. Generally, it is innovative bipolar technology, which vaporizes, I desiccates tissue. Using of normal saline (0, 9% solution of Sodium Chloride) is utilized to avoid risk of ■ ; mia, instantaneous tissue vaporization eliminates resection chips, and contact technique enables visualization of tissue effect. Electric generator enables 6 dilferent modes, such as 3 povver levels for functions, 2 combinations for vapor/cutting and coagulation and I mode for coagulation only. For \ conventional (monopolar), resectoscopy is harder to use because creates resection chips and scissors motion. Furthermore, conventional resectoscopy higher costs because of fluids and anesthesia. Moreover, it s less convenient and complicated because of electrode change-out and manual povver setting. So, the capital advantage of bipolar technology is tissue vaporization vvithout residual parts, electricity doesn t pass through the patient s body and there is more safety because of normal saline using. Aithough short for a while, our experience and results stronglysupportsthatapproach in advanced hysteroscopy.
