Purpose: Blue nevi are benign melanocytic skin lesions and histologically represent diffuse proliferation of spindle-shaped melanocytes. Erratic migration of neural crest cells through Mullerian duct, melanocytic differentiation of Schwann cells, migration of melanocytes from adjacent mucocutaneous areas, epidermidization of cervical epithelium are the hypotheses which support the origin of melanocytes in genital areas. We report three cases of blue nevus of the endocervix to describe this rare phenomenon and review the literature with an emphasis on differentiating it from other pigmented lesions, particularly from melanoma which is malignant and has bad prognosis. Methods: The clinical, gross pathologic, and light microscopic features of three blue nevi of the endocervix which were incidentally detected were studied. Special stains were performed and literature was reviewed. Results: Gross examination of all three cases showed flattened grey black lesion at endocervical region. Microscopy showed that the blue nevi were restricted to the stroma of the endocervix and were characterized by clusters of dendritic melanocytes with benign histologic features. These cells were positive for melanin. No mitotic figures, atypical features, or necrosis was noted. Conclusion: Common blue nevi are asymptomatic lesions and may be missed on gross examination. Thorough gross and microscopic examination is necessary to rule out malignant melanoma which has an adverse prognosis. |
