Prediction of DST sensitization: II. Rapid calculation of the probability of sensitization

Abstrakt: A statistical model of DST sensitization has been developed which allows calculation of the probability of sensitization of the DST recipient to the HLA‐A and ‐B locus antigens of the potential kidney donor. By linear logistic analysis, six factors were found to be associated with DST sensitization: PRA, pregnancy, previous transplants, degree of HLA‐matching, random blood transfusions and the administration of azathioprine with the DST transfusions. The logistic model incorporating the factors has been converted into a simple paper‐and‐pencil format. Values for each of the factors together with the corresponding conversion scores are arranged in look‐up tables. Simple addition of the scores gives a total score which is converted to the predicted probability. With this instrument, it is possible to rapidly calculate the probable risk of DST sensitization for selected donors, with and without azathioprine administration, and consider alternate transplant options for those patients at high risk of sensitization.
