Transplantation of cadaver kidneys from pediatric donors

Abstrakt: Information from the UKTS database regarding the outcome of 492 kidney transplants from cadaver donors less than 10 years old was analyzed. When a subgroup of donors under 5 yr old was considered. significantly better 1‐yr graft survival was found in recipients under 10 yr than in adult recipients (86% v 59% p < 0.05). Graft survival in adult recipients was similar for all donor age groups greater than 12 months. When the details from our own center of 79 adult recipients of cadaver kidneys from donors under the age of 16 yr were analyzed and compared with a similar group of recipients of adult donor kidneys, no significant difference was found in 1‐yr graft survival between any of the donor age groups, although there was an increased incidence of technical complications in recipients of kidneys from the younger donor groups. Other than a reduced creatinine clearance at 1‐wk post‐transplant in the under‐5 donor group, creatinine clearance was similar for all groups. The findings reported in this study suggest that kidneys from young donors should be transplanted to recipients of similar age whenever possible, but if no suitable pediatric recipient is available, an adult recipient should not be excluded.
