Abstrakt: •Ambivalence is most strongly associated with opting out in all groups.•Factors associated with opting-out differ by racial/ethnic group.•Healthcare mistrust is a significant opting-out factor for Hispanic participants.•More registry contact could help reduce opt-out among Hispanics.•Black donors who remember joining the registry and Asian/Pacific Islander donors with parental support are less likely to opt out.
