Background:Current legislation imposes financial penalties for institutions with high 30 day readmissions for acute myocardial infarction (AMI). BRIDGE is a nurse practitioner (NP) based post discharge transitional care program aimed at ensuring prompt (target: within 2 weeks) follow up (fu) and facilitation of care for cardiac patients. Herein we report the effect of BRIDGE on readmissions and ED visits in over 1000 cardiac patients. |
