DIAGNOSTIC VALUE OF THE SIMULTANEOUS DETERMINATION OF URINARY FREE AND CONJUGATED CORTISOL IN CUSHING'S SYNDROME

Abstrakt: The urinary excretion of free and conjugated cortisol in ten normal subjects and in twenty patients suffering from Cushing's syndrome was determined before and after adrenocortical stimulation with corticotrophin. Under basal conditions the level of free cortisol was elevated in all patients except one. The values ranged from 85 to 676 μg/day (normal 69 ± 14, range 48–89 μg/day). The level of conjugated cortisol was distinctly raised in 18 patients. The values ranged from 145 to 1048 μg/day (normal 122 ± 32, range 86–168 μg/day). In the patient with a normal amount of urinary free cortisol and in three other patients with slightly elevated levels, the excretion of conjugated cortisol definitely exceeded the upper limit. In these cases the sum of free plus conjugated cortisol reflected more accurately the clinical state of the patient and the cortisol secretion rate than free cortisol alone. From these results the investigators consider the simultaneous determination of free and conjugated cortisol as more informative than the usual determination of free cortisol only.
