Documentation of pancreatic insufficiency in children with CF is important since 20% have normal function and do not require specific treatment. Currently, there are no simple tests to determine pancreatic function in children. We have evaluated a new approach for the diagnosis of pancreatic disease in CF. The synthetic peptide N-Benzoyl-L-tyrosyl-p-amino-benzoic acid (Bz-Ty-PABA), administered orally, is specifically cleaved by the endo-peptidase chymotrypsin to Bz-Ty and PABA; the latter is absorbed and excreted in the urine. We studied: a) a control group of children and b) children with CF and steatorrhea (fecal fat > 18 gm/72 hr). Bz-Ty-PABA, 14 mg/kg, equivalent to 5 mg/kg of PABA, was given orally and urine was collected over a 6 hour period. Results were as follows (mean ± SD):There was a significant correlation of PABA recovery with fecal chymotrypsin (r=0.60, p<0.05) and fecal fat (r=-0.68, p <0.05). Urinary recovery of PABA of the peptide Bz-Ty-PABA 1) correlates with other parameters of exocrine pancreatic function, and 2) may prove a practical test of pancreatic function in CF. |
