Injuries responsible for significant vascular trauma also damage surrounding soft tissue. The mechanism of the injury is key to predicting the extent of injury and determining treatment. Failure to recognize the extent of local vascular injury is the most common cause of reconstructive failure with both local and distant tissue transfer. Primary closure is optimal in preventing wound infection but is not appropriate in the face of heavy contamination. |
