Recurring pelvic peritonitis

Abstrakt: A syndrome clinically characterized by dysmenorrhea, dyspareunia, and pelvic pain was ascribed in 1955 by Allen and Masters to excessive uterine mobility on a universal joint between cervix and fundus. The syndrome appeared to be anatomically based on a tear of the posterior face of the broad ligament and a varicocele. Twenty-five patients with this syndrome were operated upon, of which 17 showed a typical weakened area of the broad ligament of the type Allen and Masters regarded as due to a laceration and 8 had no anatomic sign. Large biopsies disclosed the same features in both series. Endometriotic lesions of a very small size were found in half of these cases and less frequently angiomas, neuromas, and evidence of cytonecrosis. Edematous exudation, fibrous organization, and lymphoid reaction were constant findings. The authors think the “tears” of the broad ligament are coincidental, secondary to other pathologic lesions, without special significance. For this condition they propose the term of “recurring pelvic peritonitis.”
