•A cluster of severe infections caused by NDM-1-producing E. cloacaestrains occurred in four patients overlapping their hospital stay in the surgical ICU of a tertiary care hospital in Pula, Croatia.•All strains belonged to ST133 and were positive for IncHI2 replicon, suggesting localization of blaNDM-1 on an IncHI2 plasmid, a possible important vehicle for diffusion of this resistance gene.•The cluster occurred despite efforts to implement infection control measures after the first case, since interventions were suboptimal.•New Delhi metallo-β-lactamase (NDM) represents nowadays an epidemiological and clinical concern worldwide.•Interspecies diffusion of this resistance mechanism to emerging pathogens necessitates the setting up of optimal joint measures to control the spread of NDM carbapenemase worldwide. |
