Chronic anterior shoulder instability was found in cases with little soft tissue on the anterior portion of the joint, especially the anterior bundle of the inferior glenohumeral ligament (AIGHL) complex. Low volume and quality of the AIGHL complex tissue can lead to limitation of external rotation and difficulties to return to activities of daily living and overhead throwing sports. A new approach for the augmentation of chronic Bankart repair was developed for these cases based on a pilot study using polyglycolic acid sheets (PGA sheet). Favorable functional results without recurrence were obtained using the PGA sheet for augmentation of the AIHGL complex without any side effects. A long-term follow-up is necessary. Eine chronische Schulterinstabilität zeigte sich bei Patienten mit wenig Weichgewebe im vorderen Gelenkbereich, v. a. im Bereich des „Anterior-bundle-of-the-inferior-glenohumeral-ligament“ (AIGHL)-Komplexes. Wenig Volumen und geringe Qualität des Gewebes im AIGHL-Komplex können zu eingeschränkter Außenrotation und zu Problemen bei den „activities of daily living“ (ADL) sowie bei Sportarten mit Überkopfwerfen führen. Für die Kombination der Bankart-Operation mit einer Augmentation wurde ein neuer Ansatz entwickelt. Basis war eine Pilotstudie, in der Polyglykolsäure(PGA)-Sheets eingesetzt worden waren. Mit dem PGA-Sheet für die Augmentation des AIGHL-Komplexes wurden günstige funktionelle Resultate ohne Rezidive erzielt. Erforderlich ist ein langfristiges Follow-up. |
