A congenital condition characterized by dilated renal collecting ducts and medullary cysts, medullary sponge kidneys are usually diagnosed incidentally by intravenous pyelography with the demonstration of radially oriented ducts and cysts ramifying from the calyces in a bristle pattern. Nephrocalcinosis, hypercalciuria, and pyelonephritis are common findings, but impaired glomerular filtration is uncommon and renal insufficiency unusual. Urine concentration power may be decreased and defective tubular capacity for elaboration of ammonia has been postulated. Two patients with this condition excreted hyperosmolal urine in response to 14-hour fluid deprivation and short-term oral ammonium chloride loading indicated no impairment of urine acidification, of titratable acid excretion, or of ammonium production. Presumably disturbances in discrete renal functions are determined by the extent and sites of cystic tubular dilatation and of associated epithelial alterations. |
