THE VALUE of the sulfonylurea compounds as effective hypoglycemic agents, with a relatively low incidence of side effects, has been well established. However, prolonged hypoglycemia accompanying the use of these agents in diabetic, as well as nondiabetic, patients recently has been emphasized.1-4 It is the purpose of this communication to record another instance of prolonged tolbutamide-induced hypoglycemia characterized by persistent high serum levels of the sulfonylurea after discontinuation of the medication. REPORT OF A CASE A 72-year-old white male was admitted to Jefferson Medical College Hospital on March 4, 1962, in a comatose state. The patient had been a mild diabetic for at least 17 years and had been well maintained on 20 units of isophane insulin suspension and diet. Six months previously, following admission of the patient for hydroarthrosis of the right knee, insulin was withdrawn and the patient was treated with tolbutamide in doses of 500 mg |
