Coagulopathy Associated With Moxalactam

Abstrakt: To the Editor.—We read the article by R. L. Pakter et al (1982;248:1100) about the coagulopathy associated with the use of moxalactam disodium. We have seen three cases recently of qualitative platelet defects associated with the use of moxalactam that we would like to report.Report of Cases.—Case 1.—An 86-year-old man was admitted to the hospital with cerebrovascular accident. He needed gastrostomy tube insertion. Results of Quick's prothrombin time, partial thromboplastin time (PTT), and bleeding time tests were normal preoperatively. Aspiration pneumonias developed and therapy with moxalactam was started. Subsequent to this, the PTT prolonged to 64 s (normal, <40 s), and the Simplate bleeding time was more than 30 minutes (normal, less than seven minutes). Platelet aggregometry showed no reaction with collagen and disaggregation with high-dose adenosine diphosphate. Ristocetin and epinephrine levels were normal.
