Metiamide therapy of bleeding peptic ulcer during chronic hemodialysis

Abstrakt: Summary An H2-receptor antagonist, metiamide, successfully controlled otherwise refractory recurrent bleeding from gastrojejunal ulcer in a patient with renal failure on chronic hemodialysis. Intravenous infusion at the rate of 2 mg/kg hr induced sustained achlorhydria, and the bleeding stopped promptly. Removal of the drug by renal excretion and dialysis was negligible. Pharmacologic H2 blockade allowed delay of gastric surgery in this fragile, decompensated patient.
