In vivo andin vitro studies of cellular immunity were performed in 32 patients with alcoholic liver disease and in 13 patients with chronic alcoholism. Eighty percent of patients with alcoholic liver disease failed to develop a delayed hypersensitivity reaction (DHR) to a challenge of 100 µg of DNCB in contrast to 11% (P<0.01) in the group of patients with chronic alcoholism. There was a significant decrease in responsiveness to SK-SD (P<0.05) and to mumps antigen (P<0.05) in the alcoholic liver disease group. All but one of the patients who were DNCB negative responded to croton oil. This makes faulty macrophage function in the genesis of the negative skin test unlikely. Elevated immunoglobulin levels were associated with the loss of DHR in 5 of the 8 patients tested in the alcoholic liver group. This is not a significant correlation. |
