Summary In 186 patients with multiple myeloma the kidneys of 170 cases were examined morphologically. In 87 cases exact data on renal function were available revealing a normal excretory function of kidneys during the last 14 days in only 12.6% of all patients, 25.6% showed a slightly, 62.0% a severely reduced renal function. Uremia was the main cause of death in 32% of the deceased. When correlated with morphological findings pathological-anatomical evidence of myeloma kidney was noted in 44.8% of all patients with renal insufficiency, 32.8% however simultaneously showed the signs of acute renal failure. In about a third (28.9%) of all insuffizient kidneys, only signs of acute renal failure were evident, in 3.9% each a pyelonephritis or a contracted kidney could be implicated as the cause of renal dysfunction. When discussing the correlation between structure and function the unusually frequent picture of acute renal failure or tubular insufficiency must be taken into account when considering therapy. Due to careful control of excretory functions, appropriate prophylaxis and timely therapy uremia seemed to be a less frequent cause of death during the last two years. |
