Hypovolemia in toxemia of pregnancy: Plasma expander therapy with surveillance of central venous pressure

Abstrakt: The influence of plasma expanders on hemodynamics of the placenta, the maternal blood pressure and kidney function was investigated in 20 patients with EPH gestosis. The plasma expanders were administered under surveillance of the central venous pressure (CVP), measured in the superior vena cava. In 18 of the patients, abnormally low values of the CVP similar to those in patients suffering from severe shock, polytraumatisation or extensive burns were observed. In the remaining 2 patients, the CVP was within the normal range due to effects of early stages of right heart insufficiency. In these cases the administration of volume expanders is contraindicated or should only be attempted with simultaneous surveillance of the CVP. When indicated, diuretics or digitalis should be given. In 5 patients, the hemodynamics of the placenta was examined. Improved placental blood flow was obtained following plasma expander therapy. The use of the complamin test in the diagnosis of pathological placental blood flow has been discussed. As further actions of plasma expanders, decrease in diastolic blood pressure and increased urinary excretion after diuretics have been observed. The patho-physiological mechanisms for these actions and their therapeutic applications have been discussed.
