Target controlled infusion (TCI) combines a real-time pharmacokinetic model with an infusion pump and allows the administration and maintenance of a constant blood concentration of drug. TCI also allows alterations in the blood concentration to be made rapidly and easily. A TCI system for propofol, the 'Diprifusor', is now available and has greatly simplified the administration of propofol for maintenance of anaesthesia. TCI systems for other drugs exist, such as alfentanil and remifentanil, but are not yet commercially available. TCI propofol for maintenance of anaesthesia offers several benefits compared to volatile agents, including rapid recovery with a low incidence of nausea and vomiting. This review explores the development of TCI and discusses its advantages and disadvantages. |
