Quality Assessment of Platelet Concentrates by Platelet Rich Plasma: A Single Institution Experience.

Abstrakt: The quality of platelet concentrates (PC) is an important issue in transfusion therapy. Various factors are found to be responsible for poor quality of PC such as pH, total white cell and platelet count as well as bacterial culture. Platelets are stored at 22°C which can lead to increase lactate production due to glycolysis and fall in pH. Presence of leucocytes and bacterial contamination of the platelet concentrates lead to competition on the available oxygen which cause rapid fall in the pH. Objectives: This study was aimed to determine the quality of platelet concentrates prepared by platelet rich plasma platelet concentrates (PRP-PC) by measuring pH, total white cell count, platelet count and bacterial culture. Design: A cross sectional study was performed from August 2008 until August 2009 at the Transfusion Medicine Unit, Hospital Universiti Sains Malaysia. Materials and method: All the platelet concentrates were randomly selected and the sample was separated into the second bag, which were presented for analysis. Samples were subjected to white blood cell and platelet count using Sysmex XT--2000i-1 and bacterial culture using the Bactec System. Daily pH measurements were measured with a pH meter until the platelet concentrates were released to the patient. Data were then analyzed using SPSS Software. Result: From 150 samples of platelet concentrates, the mean platelet count was 55.92 x 109/ unit and WBC contamination was 0.9 x 1011/ unit respectively. The pH of the platelet concentrates showed reduction throughout the five days but was still maintained within the recommended guideline. There was a significant correlation between pH and platelet count. Conclusion: The quality of platelet concentrates were maintained well within the usual 5 days thus an extension of platelet storage time is recommended. [ABSTRACT FROM AUTHOR]
