The article focuses on the significant role played by social care at the end-of-life phase of patients. It states that function of social care in meeting and assessing the needs of patients at the end of life phase is usually narrowly understood by healthcare workers. It says that social care workers have exceptional training and skills around assessment, support, and advocacy and usually have important access to patients nearing death and their families. |
