Abstrakt: Angio-computed tomography (angio-CT) and angio-magnetic resonance (angio-MR) represent valuable imaging methods in the assessment of renal donors before renal transplantation. Compared to CT, MR is able to depict all vascular and parenchymal details, lacking the drawbacks specific to CT, as radiation exposure and use of iodinated contrast agents, the latter related to nephrotoxicity and possible iodine intolerance. MR assessment of renal donors is used on a large scale to assess renal vascular anatomy, renal parenchymal appearance and details related to urinary anatomy. Using a well-defined and reproducible examination protocol, angio-MR may study the renal arteries (origin, bifurcation, number and position of renal arteries) as well as renal veins. With both unenhanced and contrast-enhanced phases (using well-tolerated paramagnetic contrast agents), MR adequately shows all types of parenchymal abnormalities related to kidney position, size, shape, structure and function. A late excretory study using a three-dimensional acquisition may provide a complete view of the appearance of the urinary ways, similar but more detailed compared to conventional iv excretory urogram. Due to its lack of patient radiation and iodinated contrast exposure, angio-MR performed on a modern MR system and with a well-established examination protocol is considered a valuable option to angio-CT in the assessment of renal donors before renal transplantation.
