The article explains how the Seniors Wellness Clinic (SWC) program in the U.S. was developed, implemented and evaluated. The program is based at the Toronto Western Hospital in Toronto, Ontario and is created for promoting and supporting a health aging process older adults against chronic diseases. Additionally, the type of patients benefiting from the community-oriented program are those having one or more chronic health problems affecting their physical and psychosocial health. |
