Contained Robotic-Assisted Ovarian Cystectomy to Minimize Intra-Abdominal Spillage.

Abstrakt: To demonstrate the advantages of intraoperative specimen bag placement prior to robotic-assisted cystectomy for improved containment of cyst contents. Step-by-step demonstration of pre-cystectomy intraoperative specimen bag placement and use during robotic-assisted ovarian dermoid cystectomy. Academic Medical Center. The case example highlights a 37-year-old G4P2022 with concern for bilateral dermoid cysts measuring 8.5 cm and 6.8 cm on ultrasound who presented for surgical management. A large specimen bag was placed in the pelvis at the start of the case. Robotic-assisted ovarian cystectomy was then performed in the usual fashion within the specimen bag. All cyst contents were contained within the bag with no spillage. The specimen bag and all the cyst contents were then removed. In the case example, robotic-assisted ovarian cystectomy was successfully completed within a large specimen bag with complete containment of all cyst contents. The use of intraoperative specimen bag placement during robotic-assisted laparoscopic ovarian cystectomy and placement of the entire ovary and cyst within the bag prior to cystectomy can be used to effectively contain ovarian cyst contents. This will lead to decreased spillage and in turn decreased risk of chemical peritonitis, adhesion formation, operative time, and use of harmful electrosurgery on ovarian tissue.
