Abstrakt: Maternal mortality is still a global concern today and it is also caused by three delays, namely delays in seeking help (type-1), delays in identifying and accessing health centers (type-2), and delays in getting the proper treatment (type-3). Describe cases of maternal death in a tertiary hospital, namely Dr. Soetomo General Academic Hospital, so that the root causes of maternal mortality can be found, and improvements can be made. This descriptive study was conducted at the Obstetrics and Gynecology Department of Dr. Soetomo General Academic Hospital, Surabaya. Data collection was taken from January - December 2022, where subjects were taken from secondary data on maternal mortality. The total number of maternal deaths in Dr. Soetomo General Academic Hospital in 2022 was as many as 912 cases. There were 53 cases of maternal death from 926 live births. Most deaths occurred in January 2022 (16.9% of all cases). Indirect causes of maternal death were the leading causes in this study, including septic shock, preeclampsia/eclampsia, and bleeding. The three models of delay are the three main factors causing maternal death, which are interrelated and influenced by other elements, with delays in seeking help and treatment (52.8%) being the highest cause of maternal death— more than half of maternal deaths in Dr. Soetomo General Academic Hospital during 2022 due to indirect causes. Among the three delay models, the combination of delay in seeking help and delay in obtaining appropriate treatment is the main factor in maternal mortality. [ABSTRACT FROM AUTHOR]
