Dilated odontoma (DO) represents the most extreme manifestation of dens invaginatus, a dental anomaly characterized by its predominantly spherical shape, giving it the name "dilated" odontoma. In this series, we present three cases of DOs with a focus on their imaging characteristics. All cases occurred in males, with an average age of 27.0 years (SD±4.5), and were asymptomatic. Two of the lesions were located in the mandible, while one was found in the maxilla. Radiographically, these lesions exhibited an abnormal shape that resembled a tooth crown, featuring a central radiolucent area surrounded by radiopaque structures resembling dentin and enamel. These structures were enclosed by a thin radiolucent halo. Treatment involved conservative surgical excision. Morphologically, the lesions were predominantly composed of dentin, with some areas resembling enamel matrix. The final diagnosis was confirmed as DO. A careful analysis of imaging findings is crucial to ensure an accurate diagnosis and appropriate treatment of this dental malformation, thus preventing potential physiological, aesthetic, and functional issues.
