A 57-year-old male patient experienced purulent discharge and pain in the right posterior mandible following the extraction of tooth 48. Notably, the patient had previously undergone radiotherapy and chemotherapy in 2006 for laryngeal cancer. After clinical and imaging assessments, the working diagnosis was osteoradionecrosis (ORN). The patient was prescribed a combination of amoxicillin-clavulanic acid and metronidazole to address the condition. Subsequently, the patient underwent four sessions of antimicrobial photodynamic therapy (aPDT) using a 0.01% methylene blue solution. An optical fiber was utilized to irradiate the affected area with a red laser, emitting at 660nm, with parameters set at 100mW, 9J, and 90 seconds (Therapy EC, DMC, São Paulo). Additionally, local ozone therapy (OZ) was administered with ozonized sunflower oil. Following these treatments, the patient exhibited a resolution of purulent discharge. Photobiomodulation (PBM) with a red laser at 660nm, 100mW, 4J, and 40 seconds was subsequently introduced. The patient experienced complete wound healing and has been closely monitored for four years. Overall, it appears that a combination of aPDT, PBM, and OZ proves to be an effective adjuvant approach for addressing ORN in this case. [ABSTRACT FROM AUTHOR] |
