A 72-year-old male visited the clinic due to a radiographic finding. The intraoral examination did not reveal any clinical abnormalities, and the patient did not report any pain. The radiographic examination revealed a radiolucent, unilocular, well-defined lesion with a radiopaque halo, situated in the right body of the mandible, measuring approximately 2 cm in diameter. The initial diagnostic hypotheses included odontogenic keratocyst or residual cyst. Subsequently, an enucleation procedure was performed. The histopathological evaluation showed multiple fragments of a cystic wall composed of fibrous connective tissue, with focal inflammation, and lined by stratified squamous epithelium, ranging from three to nine cell layers thick. The epithelium predominantly exhibited an orthokeratinized surface, a prominent granular layer, and a basal layer of cuboidal cells, which were oriented flat over the basement membrane. The cyst contained abundant keratin production, forming multiple lamellated sheets inside the lumen. The final diagnosis was confirmed as an orthokeratinized odontogenic cyst. The patient is currently under clinical and imaging monitoring, and there have been no signs of recurrence. [ABSTRACT FROM AUTHOR] |
