MIXED LESION IN MAXILLARY SINUS ASSOCIATED WITH RHINOSINUSITIS AND PERIORBITAL CELLULITIS: AN INTERDISCIPLINARY APPROACH.

Abstrakt: The case involves a 58-year-old female who presented with periorbital pain and edema, ophthalmoplegia, along with the presence of purulent discharge in both nasal cavities. A facial panoramic radiograph and tomography revealed a radiolucent lesion surrounded by a well-defined radiopaque halo in the right maxillary sinus, leading to periorbital cellulitis and complete opacity of the frontal sinus. In the initial approach, a transmaxillary sinusotomy was performed using Caldwell-Luc access. This involved cyst enucleation, wide resection of the maxillary sinus walls, and orbital access on the sinus roof to drain the purulent discharge. Additionally, frontal, ethmoidal, and sphenoidal sinusotomies were performed on the right side. Histopathological examination confirmed the presence of a mucous cyst. In the subsequent approach, a new drainage procedure was conducted to address the periorbital purulent discharge. This involved supraorbital access and orbitotomy, along with the resection of synechiae within the right nasal cavity. The patient continued with antibiotic therapy and follow-up consultations.
