SURGICAL TREATMENT OF OSTEORADIONECROSIS ASSOCIATED WITH L-PRF MEMBRANES: CASE REPORT.

Abstrakt: Osteoradionecrosis (ORN) is a severe complication associated with head and neck radiotherapy, and its management remains quite challenging. Recent studies have suggested the use of platelet and leukocyte-rich fibrin membranes (L-PRF) for both the treatment and prevention of ORN. In the case of a 68-year-old male patient diagnosed with tonsillar squamous cell carcinoma, who had previously undergone chemotherapy and radiotherapy, significant progress was observed after three years. He developed a periapical lesion in relation to teeth 46 and 47, which necessitated endodontic treatment. As the bone lesion continued to worsen, the patient was prescribed pentoxifylline and tocopherol, and this regimen was maintained for over one year. However, after this duration, the patient reported the spontaneous avulsion of tooth 46, which was accompanied by bone exposure and pain. As a result, a treatment plan was formulated, including the exodontia of tooth 47, bone debridement, and the application of L-PRF membranes. An anatomopathological examination confirmed the presence of ORN. Six months following the surgical procedure, the patient exhibited satisfactory healing with no indications of infection or pain. Consequently, L-PRF stands as a promising option for the treatment of ORN. [ABSTRACT FROM AUTHOR]
