Abstrakt: Unicystic ameloblastoma is often initially treated as a cyst, and the correct diagnosis is typically established only through microscopic analysis. The optimal treatment method remains a topic of debate in the literature, with options ranging from enucleation to partial resection. In this study, we present the case of a 30-year-old female patient diagnosed with mural proliferation unicystic ameloblastoma in the right mandibular body. The initial treatment involved marsupialization, which led to radiographically confirmed tumor regression. However, after three years of the patient abandoning treatment, she returned, and another marsupialization was performed, considering the previous successful regression of the tumor. Surprisingly, after 8 months of decompression with no signs of regression, the surgeon decided to opt for partial mandibular resection, followed by immediate bone reconstruction in two stages. This study also includes a brief literature review discussing treatment modalities for unicystic ameloblastoma, emphasizing the importance of considering the type of tumor proliferation, its location and size, as well as the patient's age when determining the most suitable treatment approach.
