In a hospital setting, the dental surgeon plays a crucial role in aiding the diagnosis and management of oral manifestations of hematologic disorders, such as Immune Thrombocytopenic Purpura (ITP). A 23-year-old woman sought care in the Emergency Department of a University Hospital due to gingival bleeding. The bleeding had commenced after a scaling procedure performed a month ago by a dentist from Primary Health Care (PHC). Upon physical examination, we observed swelling and widespread purplish gingival hematoma, along with active bleeding in both free and attached gingiva. Local hemostasis was successfully achieved through tranexamic acid rinses. Hematological examination (HCT 16%; Hb 4.9% and PLT 2000) raised suspicion of ITP, an autoimmune disease characterized by a low platelet count, which typically results in self-limited bleeding. A differential diagnosis with leukemic infiltrate was also considered. The dental surgeon can significantly contribute to the diagnosis and treatment of the oral manifestations of ITP.
