Lichen planus and lichenoid lesions affecting the oral mucosa often present very similar, if not identical, clinical features. The objective of this discussion is to present a clinical case that can assist clinicians in the process of differential diagnosis and management. In this study, a 69-year-old female patient exhibited symptomatic white plaques and erosive lesions primarily on the lateral mucous membranes of the tongue and cheeks. A biopsy of the lesion on the lateral tongue was performed. The results confirmed the diagnosis of lichen planus. Furthermore, the pathologist noted a prior skin examination that had yielded a diagnosis of lichen planus. The patient was educated about the condition and referred for a medical evaluation. In conclusion, this case study underscores the importance of conducting a thorough review of a patient's medical history and highlights the significance of anatomopathological examination for both skin and oral mucosal alterations in the diagnostic process.
