CASE REPORT - UNUSUAL ASSOCIATION BETWEEN PARACOCCIDIOIDOMYCOSIS AND TUBERCULOSIS.

Abstrakt: A 46-year-old man presented with a persistent unproductive cough, weight loss, and buccal mucosa pain that had been ongoing for one year. During the medical history assessment, he disclosed a prior tuberculosis diagnosis with treatment having been completed two months earlier. Upon extraoral examination, multiple deep ulcers were observed on the forehead, forearm, and leg. The intraoral examination revealed multiple moriform ulcers throughout the oral cavity. Based on the clinical assessment, a decision was made to perform an incisional biopsy with the suspicion of paracoccidioidomycosis. Microscopically, well-formed granulomas featuring multinucleated giant cells, lymphocytes, and multiple budding yeast forms with birefringent walls (PAS and Grocott-Gomori positive) were observed. This led to the diagnosis of oral paracoccidioidomycosis. The patient has been receiving Itraconazole therapy for one month, and the oral lesions have shown improvement. This case report was reviewed by an infectious disease physician who confirmed tuberculosis through immunochromatographic testing and sensitivity analysis to identify "Mycobacterium tuberculosis."
