The dentigerous cyst is an odontogenic cyst that typically associates with the crown of an unerupted permanent tooth. A 38-year-old male patient was referred by the orthodontist due to a bone lesion in the region of tooth 38. The patient reported numbness and tenderness in the mandible. Imaging exams revealed tooth 38 in an inverted vertical position and a radiolucent lesion encompassing the crown, starting from the cementoenamel junction and extending to the mesial root of tooth 37 and the base of the mandible. Consequently, the patient underwent enucleation and curettage, along with the extraction of tooth 38 associated with the lesion. Histopathological examination confirmed the diagnosis of a dentigerous cyst. After a 24-month follow-up, bone regeneration was observed, and the patient remained free from complaints or signs of recurrence. Despite being a benign lesion, it can attain significant dimensions and weaken the mandible, as observed in this case, leading to atypical symptoms such as numbness in the affected region.
