Paracoccidioidomycosis is a fungal infection more commonly found in Latin America and is a significant public health concern in Brazil. This paper presents a clinical case of a 60-year-old female patient who complained of severe oral pain persisting for two years. Upon physical examination, ulcerated lesions with a moriform appearance were observed, primarily affecting the ventral, dorsal, and lateral aspects of the tongue. Given the patient's decompensated systemic arterial hypertension, it was decided to perform an exfoliative cytological examination instead of an incisional biopsy at that time. Cytologic smears revealed the presence of epithelial cells and multiple round structures consistent with P. brasiliensis, as evidenced by Papanicolaou staining. Consequently, exfoliative cytology of the oral cavity proves to be a readily accessible test, serving as a valuable complementary method for diagnosing oral lesions related to paracoccidioidomycosis.
